Why do people who borrow money from you, make you feel bad when they pay you back?

When they offer it back, you say. 'Are you sure your okay'! and they respond with something like,'I should be okay, if i don't eat or pay buy petrol for the car or not take the dog to the vets, or buy my daughter a wheelchair... .....etc'.
